ILOTA AWARDS NOMINATIONS 

Award of Appreciation: expresses appreciation to ILOTA members or individuals who are not OTs for extraordinary contributions to the advancement of our profession in Illinois.

Award of Merit: honors a member of ILOTA whose actions have shaped the course of occupational therapy and thus contributed significantly to its growth and development of Illinois.

Board Award of Merit: recognizes members of the ILOTA Board who have made outstanding contributions toward facilitating board business.

OTA of the Year: recognizes the outstanding contributions of an OTA

.OT of the Year: recognizes the outstanding contributions of an OT.

Public Awareness Award: recognizes members of ILOTA and non-members who have increased public awareness and understanding of occupational therapy through the media or in the political arena.

RA REPRESENTATIVE APPLICATIONS BEING ACCEPTED!

Representative and Alternate Representative Nomination Form

Nominees are solicited and vetted by the State Affiliate with the election conducted by the Association. As directed by Association Bylaws and further defined in the Nominating Committee’s Standard Operating Procedures (SOP), the Nominating Committee oversees the nomination process.

Nominees must have consistent access to a computer with high speed Internet connection and be comfortable working in e-mail environment and other virtual communication environments. This includes opening, saving, editing, and sending attachments in e-mail and other electronic formats as needed.

Criteria for Representative and Alternate Representative

1. Be an occupational therapist (OT) or occupational therapy assistant (OTA).

2. Be a member in good standing of AOTA and the election-area affiliate at the time of the election and throughout the term of office.

3. Be a voting member of the election area to be represented at the time of the election and throughout the term of office.

4. Maintain any election area regulatory requirements necessary to identify themselves as OTsand OTAs throughout the term of office.
